What do the leaves look like? And where are you located (State)?
I just looked through “The Ortho Home Gardener’s Problem Solver†book and couldn’t find anything that resembles your description… “Scab†leaves “olive-brown velvety spots†on both the leaves and young fruit, but doesn't have the “deflated†look or orange color you’ve described.
They may have withered from malnutrition and are simply decaying via the fungus?
…I was also scanning my book for eighth inch round raised ‘green blister’s’ on the leaves of a friends pear tree. It has some scab on the fruit but thinning will remove the bad ones. I’ve never seen such blisters; they haven’t turned color and don’t look to be insect induced...

It's a fungus.
The Juniper is the host for a type of sickness called "pear rust" over here. The insulting organism is Gymnosporangium fuscum (or sabinae), a fungus.

I have had some pears for six years and this year, thanks to a new neighbor who planted some diseased cedar trees and then neglected them, had rust for the first time. I purchased a product called Serenade (active ingredient - QST 713 strain of Bacillus subtillis) from Portland Nursery. It took two applications, but the rust stopped spreading and no new rust has appeared. Hope this works for you.

I know that there are some cedar-type trees as a privacy screen in my neighbor's yard. They look healthy, but I wonder if this is a foreshadowing of problems the trees will face. If those trees die, I'll be looking right into my neighbors house!

I took a class from Home Orchard Society when I moved to Oregon in 1996. In our first house, we had a several orchard trees and had to learn how to care for them. I learned a lot in one evening.

Willamette ORganics no longer makes retail compost tea. They are strictly a wholesale, farm and large operation.
The Garden Spout apparently sells compost tea. 503 788-Grow.
Also there's a guy named Terence Dodge in NE Portland who makes great compost tea too.
